In the past, most pharmacies were founded based on personal knowledge, experience and subjective judgment, without any support from effective evaluation methods regarding success factors. For this reason, it is particularly important and valuable to establish an evaluation method of important factors & indicators for the pharmacies. Through literature review and expert interview, this paper selected and concluded 4 major dimensions and 20 evaluation indicators for pharmacies, and then set up a qualitative and quantitative evaluation indicator model using Fuzzy Delphi Analytic Hierarchy (FDAHP) and Gray Sequencing, thus providing a reference for the evaluation of Taiwan pharmacies.
