This article proposes a DEA model with preference on fuzzy sets, and translates relation between fuzzy sets into the general linear relation through Campos index. Then a fuzzy DEA model based on Campos index (FBCC-CI) is put forward to measure the technological efficiency score and fuzzy efficiency score of each decision making unit (DMU), as well as to identify the efficient DMUs and to get fuzzy frontier constructed by a set of frontiers under different preference, which discloses explicitly that the derivation from the frontier is attributed not only to inefficiency but to the impact of imprecise factors. Finally, this model is proved valid by taking simplified FBCC-CI model as an example.
